been discharged and assessment coming up

HI everyone that reads this I have awful borderline personality disorder and selfharm, I used to be under the mental health team for groups and talks dbt/cbt but the last two years I have been under my doctor seeing her rather then the mental health group, but recently I have been very low and also have my esa assessment to as well to see if I still or  I am on the right benefits, that at the moment terrifies me, but anyway my doctor wrote to the health team because she thought it was time that I was referred back to them for help and maybe change my meds, but after 2 hour assessment, they decided that I needed to go to the recovery workshop,  and that was it no offer of help even though I am self harming badly again, been trying to call them and never return my calls and now I find out I have been discharged from them and have to go back to my doctors, what  the hell do I do now I am at my wits end being left on my own again I have hurt myself, and I really need some helpa and advice
