Social Services Needs Questionnaire

I have a questionnaire about my needs from Social Services, which I take as a Good Sign

However, I find it is framed to give insufficient coverage of my weaker areas e.g motor planning and organisation. A person from personnel at my old workplace said I should refer myself to them, perhaps that was not a sensible idea?

When I last filled in a similar questionnaire Social Services replied that unfortunately my needs weren't enough to meet their action criteria which naturally are under great strain these days.

Are there alternative sources of help I could enquire into?
